Using pages with relevant condition for groups, Only displays first group

Hello, I am trying to configure a survey form with 3 groups set to pages.  Right now I have a question asking "Type of Form Needed" then each group is set to a relevant condition based on the answer.

When I select the first form on the list the correct information from the group appears.

However, when I select the other 2 types nothing appears?

Also to note that it works in the Survey123 Connect but not on the phone or desktop app.
