Survey will submit in browser but not in field app

I have a survey that will submit in the browser version but when I try to submit the same survey in the field app on my computer I get an error that it was able to send. When I look at the log I see this error:

"code": 400,
"details": [
"'edits' parameter is invalid",
"An item with the same key has already been added.",
"Unable to apply edits."

The survey used to work fine in the field app until I added one select_one question and added a conditional clause to some of the questions. 

These are new records I'm submitting. I am using 3.16 for both Connect and Field app.

What I'd do is re-publish it today because last week, we had some wonky things happen to our surveys, but Esri has been working through the issues, and you might find that this got fixed too. If that doesn't work - I'm DMing you to see if we can get together to look it over Wednesday night.
