Hello ESRI community

I have a problem with my Survey123 forms, the subject itself is clear, when I go to any Survey123 that I published and owned and try to export a sample report or generate a report or even export raw data, after some seconds, it shows me 404 - File or directory not found as demonstrated in the screenshots attached. The case has been like this for a 2 weeks or more (not sure), and I need to export reports for my work necessarily.

Knowing that the forms have been published since 2022 and been exporting reports and raw data successfully all through the years until recently.

What could be the problem?

@RedhaAl-Obaidy

I would try to export different formats to see if they all are broken, or just one specific file type.

I haven't seen this error before though.

Cheers,
Jon

I would also suggest this. I've had specific file formats fail while others succeed.

I just tried this myself (Enterprise) to see if I also got the same error. Tried CSV and Excel. No issues.

I have reports being generated almost non-stop, and have not had any issues there.

Unless this issue is isolated to AGO (possible, but seems odd for this service as I thought it lived in the AGO sphere for both Enterprise/AGO users), it may be environment-specific. I did have a random access issue popup last year out of the blue, and the resoution was to add exclusions in Portal/Server Admin.

Is the survey published from the web designer or Survey123 Connect?  Is your data a hosted layer?  Is the data in ArcGIS Online or ArcGIS Enterprise (and version)?

If this just started happening 2 weeks ago, that's around the time that ArcGIS Online was updated (around June 25th), so this could be related to that upgrade.

i'm getting the same when trying to export to an excel file, my survey was published with Survey123 connect and its a hosted layer in ArcGIS Online.

can you both check that Export data from hosted feature layers or hosted tables is enabled?

i also found this other post you may want to follow - June 2026 ArcGIS Online Update – Survey123 Report Generation Issues

I have also reached out to a developer to see if they're aware of any related issues logged.  I know a Survey123 website update is planned for around 7/21/2026, so hopefully we have answers and solution by then.

hi @ToddW_stl it wasn't on the main feature class but is on the results feature class. i enabled it on the main feature class and get the same issues. in the end I've resorted to downloading directly from the feature service which is okay for me as the owner but not for the rest of the team unfortunately.

did the developer mention anything?

thanks
Stu

not yet - but it is UC week.  I seems like it's hard for others to reproduce.  Do reports work on other surveys you may have?  Can you create a new simple survey and test reporting?  If it works on other surveys, would it be too disruptive to re-create or re-publish the existing survey as a new one?
