How can I link several Survey123 forms to one point? For example, a field staffer is required to perform annual inspections of stormwater outfalls. One point - same form - multiple surveys.

I'm thinking of doing something similar for bridge inspections.

Do you have any templates that you could share to get me started?

As Gee suggested, would you be so kind to share with us a simple XLS form that properly segregates the parent information from the repeat. I am pretty sure our issue is associated with incorrectly setting up the repeat function in the xls form.

Hi Gee/Jonathan,

Repeats are added to your survey using the begin repeat and end repeat question types. These tables must be related tables to the parent layer. If you are looking for examples of how to use repeats, please take a look at the Sample available in Connect (as below). Alternatively, the Survey123 documentation: Repeats—Survey123 for ArcGIS | ArcGIS and Prepare for editing existing survey data—Survey123 for ArcGIS | ArcGIS has more information on using repeats.

I am interested in this same question - 1 Polygon - 1 Form - Multiple Inspections. 

Is the best way to accomplish this still in seeking for the specific previous form from a list and editing it?

I am more interested in implementing this from a spatial perspective by clicking on the polygon from the Collector app and having it link to that specific form with the repeats. 

To clarify:

      -1st section of initial form prompts you to make a geoshape along with basic site information (address, company, site name, etc), which when complete populates in the Collector app.

      -2nd section of form would be a repeat group - for repeat inspections for example.

      *Once the geoshape it is created, the form links back to itself. 

Anything like this possible?

Hi Kevin,

Yes, the steps you have outlined are the correct way to configure a survey to collect multiple inspections on one feature. The parent layer contains the feature (point, line or polygon) in your survey, and the repeat (related table) will contain the inspection information that will be repeated.

Thanks for your timely response Philip. 

I'm glad to see that my planned implementation is built in the correct way. My only hold up currently is figuring a way to make the parent layer[polygon, site description] link to its own repeat/related table [inspection] within Collector. 

Ultimately I would like my collector 'inspection' map to only display active, non-finalized inspections.
