Hello,
I recently republished a survey with some small changes and noticed this issue with the geoshape field - it seems to be creating a duplicate of the map window below it as a blank box. I am using Survey123 Connect's Standard Template, last modified 9/16/25.
Below is an image of a dummy survey I made with just a geoshape question without any formatting except being required and a second example question from the template retained. I've uploaded the XLS Form for you to test.
Any idea what I can do to remove this box? It is showing up in both Chrome and Edge browsers.

Unfortunately still not working with Survey123 connect 3.25 and ArcGIS Enterprise 11.3 with its own Survey-Webseite installed.

Have you set your Connect to use the installed Survey123 REST API? If you are using the online endpoint instead of the installed endpoint, the survey will be published with version 3.25, while the installed site is only on 3.24. The older website/web app may not support the newer survey.
Please refer to the "Disable other internet requests" section in the KB article below, then republish the the survey using the installed endpoint: https://support.esri.com/en-us/knowledge-base/how-to-configure-an-installed-arcgis-survey123-website...

Connect is set to use the online endpoint (https://survey123.arcgis.com/api)
If I take a look a look at the settings of a newly created survey with Survey 123 connect inkluding the bug, it says:
Both options include the bug in the web app.

I think this is the issue . Your survey is published using the online endpoint (version 3.25), so the survey is on version 3.25. However, your installed website is on version 3.22, which cannot properly render the form with version 3.25.
I think there are 2 possible solutions:
1. Update the Connect endpoint to your installed Survey123 REST API endpoint and republish the survey (perferred).
2. Republish the survey using Connect 3.22.
The 3.25 installed website is currently planned for March May.

It depends on the versions of the websites installed in all portals. If they are all on the same version, then you can use one endpoint for all portals. However, if they have different versions, you may need to use the endpoint that is oldest.
