I've been in the process of republishing a new version of a form we've used for the past two years. Up until last week, the form appeared under Download Surveys in the Survey123 app and I could download it straight from there. Now, I can't see it and the only way I can download it is to scan the QR code or open the link directly. I've gone through and checked all my sharing settings on the form, the feature service, and the map and everything seems to be fine. Anyone else have this issue?
Is it pulling any surveys or is it just omitting the one?
It shows every survey we have in our Organization except this one. I also tried creating a brand new form using this one as the template and it shows up just fine. I'm assuming something got corrupted. The bad part is I've built a couple of web maps and apps around this layer already and deleting/republishing means I'll need to redo all those...
For context, we're on Enterprise 11.1, not AGOL.
Ah yeah - I would then review the .iteminfo, item.json and .info files that are located in the My Survey Designs folder if using connect. Their could be some issue with those files.
Otherwise, I would turn on logging in the desktop field app and then try the query again. The logs may give you some insight into why it's not showing.

Did you make any large changes to the survey? E.g., did you see a message similar to this one?
If so, not only does this reset collected data in the Feature Layer, this also resets security permissions. Go to the Surveys Site (or Portal - I just prefer the Surveys Site for this particular task) and check that the survey is shared properly.
