Error publishing survey with ArcGIS Enterprise

I have deployed Survey 123 on our enterprise portal (version 10.4.1) using the following guide,
How To: Install and configure the ArcGIS Survey123 website
and the configuration was also successful,
After going to the website and creating a sample survey, when we proceed to publish, it gives us the error (-1: CONT_0005: Item does not exist in this folder.),
The folder also appears on the Portal website,
What am I missing here, and how can we resolve the issue?
