Error Message 404 when attempting to generate Report from Report(Beta)

Hi all,

I am having problems when attempting to generate a template using the Report (Beta) function within Survey123 .

the error message I get is 

Http failure response for https://survey123.arcgis.com/api/reportMerge/defaultWordTemplate: 404 OK

I have attached the xlsx file if that helps.

This is occurring on a few of the reports I am attempting to generate

Any guidance would be greatly apprecitaed

Hi all,

I don't know if someone did something or alter something but it all appears to work now. The difference being the report(beta) now appears on the side and allows me to generate a report
