For a few weeks, I have been receiving this error when I open my survey in webform?

Has anyone ever had this error?

I'm using the latest version (3.11) with this configuration: https: //survey123.arcgis.com/?portalurl=...

I'm also getting this same error message when trying to edit a survey on the web form. Has anyone figured out the cause of this?
