Create option for relevant questions to always take up space in the grid

Within a grid, create an appearance or other option that will force a relevant question to take up a space in the grid, regardless of whether the dependent question has been answered so that it satisfies the relevant condition.
I came across this problem while creating the survey in the image. I had a number of questions where if the user answered "Action Needed" I wanted them to explain it in the relevant details question. However, because of the grid behavior the questions all fit side by side, and if you answered "Action Needed" in column 2, it would bump the relevant details question to column 1 on the next row. The only options were to always have the details question available or use this great workaround provided by Brett Stokes:
