We have a Quick Capture app that allows the capture of up to 5 photos which are associated to the capture point.
5 photo's are taken, the form is completed and then saved.
When the field capture is synchronized, the last photo taken is uploaded 5 times.
Does the same on various Android Devices and iPhone.
We built a test app using an Esri template, got the same result.

Replying to add traction. Facing the same issue where the last captured photo is duplicated. Started happening 1 day ago (18 March 2025) to all my similarly configured QuickCapture Projects.
We opened a case on this with Esri and they were able to replicate the issue and it has been escalated. Hopefully the fix comes through soon as this impacts quite a few things within our organization. Will update once we know more.

Is there any way to retrieve the photos that were not attached properly? I had a colleague take many photos during an annual inspection, and many of them are lost because of this issue.

I was not able to retrieve the photos. I reached out to the Quick Capture team via email, but never heard back...
