OD Cost Matrix analysis drive time issue: missing possible records in lines

I am seeing something very strange after origin-destination cost matrix tool is run. I have set a 10-minute travel time constraint on my origins. I have 722 distinct origins (centroids) and 5465 destinations (childcare centres) most of which are too far away to be considered in the model given the travel constraint.
In my service area, there is one service centre, Y3, which provides service only to the origin X1 marked by blue circle despite other origins being nearby which require less than 10 minutes to get to the service centre circled by red, Y3. In other words, there is only one record in the lines layer where Y3 is the destination in this dense downtown urban network.
What could be the possible cause of this issue?
