Append Changing Spatial Location

I am trying to do a simple task of adding XY data and then appending the data into our Enterprise geodatabase.  Everything looks great.  Then I run the append and it completes successfully, however the location of the new point is not correct.  This is happening to all new points.  I have confirmed all datasets and maps are in the same coordinate system. When I manually plot ("Go to XY") in Pro, it plots at the same point of the XY data.  So it is the Geodatabase table side that isn't accurate.  What could possibly cause this to happen?

I get the same results with both our test and production enterprise geodatabase.  I've tried in ArcGIS Pro 3.2, 2.9 and ArcMap 10.7.1.

If i create a new feature class in the enterprise geodatabase with this data it works correctly.  Is there just something corrupt with the feature class?

With such a slight offset, I wonder if this is a transformation issue?  You can read more about it here - Specify a datum transformation—ArcGIS Pro | Documentation
