Multiple features showing up in attribute pop-up?

I have a mapservice setup on my ArcGIS Online map and having problems setting up the pop-up correctly.  Currently when a line feature is clicked on the attribute pop-up displays all of the features (example 1 of 50 with the ability to scroll through attribute pages for each feature within the feature class).  What I really want is to only have the attributes shown for the feature that is clicked on, not ALL of the features.  Has anyone else experienced this or know how to resolve the issue?

I am trying to figure out why multiple attributes on two pages show up after I create a custom attribute display.
The attributes are for grocery stores' customers.  I created a custom attribute display for the grocery stores
and I got the desired result of a one page display.

I created a custom attribute display and everything worked fine until I changed the symbol
and there were multiple attributes, two pages.  I just want one page of attributes that I selected to be displayed.

What's the status of this? I currently have a map service added into an ArcGIS Online map, and am only getting a single feature when there should be multiple features returned. It's not a feature service.

The unique thing about my map service is that the dataset is an X/Y event theme based off a dynamic database table. This allows the features to update automatically as the table changes.

Any help would be appreciated!
