Returning result for attibute search

I'm hoping someone can help me with a program to return values based on an attribute value search.

I've attached a ZIP file that includes the original, working HTML document from the forums.  The second file is my attempt to do the same with my map service.  I simply replaced the example feature service with my feature service and substituted in the attribute names for my feature service.

When opening the second file, it displays just the base map - none of the additional features I want to use for the search.  Doing the search does not yield any results, but there is not an error message.

Any assistiance is greatly appreciated.

The issue are
1)  the service URL which you have given in the example https://services.arcgis.com/iv9qLyn1eiZEZlDz/arcgis/rest/services/WorkOrder_Poly1/FeatureServer/0 is not correct it should be like https://services.arcgis.com/iv9qLyn1eiZEZlDz/arcgis/rest/services/WorkOrder_Poly1/FeatureServer a map server URL and not feature service URL
2) The service is not configured for the Find Task.At the bottom of the service it should say supported Find as one of the item as
like this [ATTACH=CONFIG]23370[/ATTACH]
you can configure that in the server manager.
