Geocoder with custom service arcgis 10.1

I followed the steps for creating an address locator, and I have published as a geocoder service when I test to find an address in ArcMap service works fine but when I want to use this service as a link in the Widget geocoder described in this  sample, I do not get the resulatat research.

in the browser developer tools do you see a geocoding request being fired?  it would be helpful to know whether the request fails or just doesn't return any valid results.

can you post a code snippet or a link to a simplified application (perhaps using something like jsfiddle)?
