Template is showing up in app instead of Form

I created a Form in Field Maps and when I open the app and try to add a feature, the template is showing up instead of the form I created.  How do I set it up so that the form is showing up in the app instead of the template?

Normally, you select add feature, select the template for the feature you want to add (so it knows what type of feature you are adding), then the form should come up.

Hello - When I do that all that comes up is the template.  I don't even have the option to pick the form I created?

Can you include a screenshot of the template you are seeing?

My template shows like this (I only have two items Sign or Post):

If I select (touch) Sign, it opens the Sign form, likewise with the New Post.

Thanks for the reply!  I actually got it working.  I was going to try one last thing before I deleted it.  What I did was go in and delete the template options that kept showing up instead of my form.  Once I did that the form was visible.
