We are in the planning stage of upgrading for our ArcGIS Server from 10.1 to the LATEST version.
Here are our current configurations:
DEV/TEST - 2 machines (1 for cluster/load balancer)

Q1) If we would like to use Amazon cloud, can we use our existing ArcSDE (oracle based, hosted in-house)
A1) While this might be technically possible, this is definitely NOT a recommended deployment pattern. You would likely get poor performance and will have to deal with security/firewall issues between your hosted in-house enterprise geodatabase and your ArcGIS Server in Amazon instance. I recommend that you migrate the geodatabase into the Cloud, see help topic:

Q2) Is there a first time setup fee charged by Amazon?
A2) You will have to get an Amazon Cloud account set-up before you can use the ArcGIS Server Amazon Machine Instances (AMIs). I suggest you reach out directly to Amazon to discuss costs.
Q3) Do we need a load balanacr if we are using Amazon?
A3) This will depend on your deployment specifics. Please review this help topic:

Q4) Is there ongoing maintenance and support cost charged by Amazon or ESRI?
A4) With respect to ArcGIS Server in Amazon, you would likely pay annual maintenance - but it depends on how your Esri software is licensed. Please discuss with your local Esri account manager/Distributor for details. Regarding Amazon Cloud costs, please reach out directly to Amazon.
Q5) What’s the traffic and data usage cost for both the ArcGIS Server and ArcSDE access hosted by Amazon?
A5) Again, this will depend on usage, traffic, how much content you plan on storing in the Amazon cloud, etc. There are many factors involved - please discuss further with your local Esri account manager/Distributor. This video is a little old, but may provide some insight with some use cases,
