If the featurelayer is set to AutoSave=false and then drag/drop a document into the AttachmentEditor, the attachment will be saved automatically. I would expect to need an explicit Editor.Save or FeatureLayer.SaveEdits. Would it not be better to have a Save button on the AttachmentEditor. This will avoid unnecessary database pollution in the case the user makes a mistake.

AutoSave only applies to the actual features saved. Attachments are not considered a part of this. Also you cannot add attachments to features not yet saved to the server.
