Sample Data Attributes - duplicates fields

When I view an item's details, the sample data attributes duplicates some of the fields.  The fields that are duplicated also do not display any sample values.  I'm wondering if it is related to having null values in the data.

A second issue I am having is the Dataset Map widget is hanging when I try to add one to may site.

I have since republished my data overwriting the previous service.  The issue with duplicate field names seems to have been resolved.The reason I republished was that I did not want the GlobalID field to be a part of the service.  I had the field visibility off, thinking that would be sufficient.  As it turns out, I needed to delete the field from my file GDB.
