Proxy Page & Dynamic Token Generation Query

Having to implement a proxy page set-up for first time & i note 'various' options when it comes to its configuration for use to access secured map services via token security.  My preferred solution is to use the proxy page setup that supports dynamic token generation (see Dynamic Token Based Authentication).  My query is why this option isn't more widely included/encouraged in the latest proxy down-loads (which vary suprisingly between apis?) & if a token is generated dynamically, what is its default timeout value?

Further to my original post, i have found while the dynamic proxy page solutions works fine in a IIS 6 environment, in an IIS 7 i was unable to get it working.

I also have some queries around the SL proxy page solution (proxy.zip via http://help.arcgis.com/en/webapps/silverlightviewer/help/index.html#//017700000046000000). 
  -  What is the default timeout unit when using this option - one presumes shortlived (hence minutes?). 
  -  Further to this, is there any recommendations around setting this value?  Given each request is a new request, presumably a lower value (5 as per sample provided) makes good sense?
