Check out these tools which export data and include attachments:
Extract Data—ArcGIS Online Help | ArcGIS (ensure output format is FGDB and Clip Features is unchecked)
Analysis widget—Web AppBuilder for ArcGIS | ArcGIS (web appBuilder widget which uses extract data tool)
Export from item
Use layers—ArcGIS Online Help | ArcGIS
