I am using the web designer feature to build a survey, but when I try to add an image the 'Click here to upload file' window doesn't open so I can upload the photo. I am using Chrome and Windows 7 and the image is only 2.62MB. Could this be on your end? Thanks

Yes it is still an issue. The weird part is The 'Add image' opens in
Preview mode and I can upload a picture, but with no way to save it. In
'Design' mode the window won't open, nothing happens actually.

Hi Andrew - it is expected that you can't upload an image in design or preview modes.
Can you confirm if you can submit the image when viewing the actual survey?

I was hoping to use the image as a reference point for the students as
the begin the survey. For example, a trail junction as a marker that
they could view and then look for a marker where they would start
collecting data for the survey.
I may be thinking of this in reverse. I haven't dropped the image tab
into the survey for them to use in the field. I was hoping I could
load an image to aid them in locating a vegetation plot to begin data
collection.

In that case I would put a note question above the image question. The note question can include text and images. To add an image, use the insert image button and specify an image url. The image will need to be hosted online - but you can use something like Flickr to host it.
