Error message when I try to run basic viewer application

Has anybody got this or seen this before (see attachment below)... If so what i am doing wrong. I made sure everything was entered correctly plus I have successfully opened other application templates before. is the basicviewer template that you download corrupted?

Is the web map shared appropriately?
Is the application published to ArcGIS.com or have you downloaded the template and deployed it to your own web server? If you downloaded it, you probably need to configure the proxy. See the readme instructions.
