It looks to me like you're performing the join backwards. In the Open() method, you pass in the table reference and its key field name first, followed by the feature class reference and its key field name. This joins the table to the feature class. Also, it looks like you're setting the definition query on the table? Shouldn't you be setting it on the feature layer?

All 3 of those features are visible; it's just that the merged feature is drawn last and hides the two smaller features that are in the same location. There's no real way to tell which feature is "on top" but from my own observation it always appears to be the feature with the higher ObjectId. My guess is that ArcMap renders the features in the order of their ObjectIds but I'm not certain that is always the case. If you need to be able to identify the merged features from the original features within a feature layer then I would suggest adding a field to the layer's attribute table that holds this information. You can then add an attribute query to your spatial filter in order to get the results you are looking for.

You can't. The M and Z aware properties have to be set at the time you create the feature class in the geodatabase. Once a feature class has been created you cannot change its geometry type.

That makes the point geometry z-aware. It doesn't make the feature class z-aware. The feature class is made z-aware when you create it in the database.

Since you're setting z-values on the geometry my first guess would be the feature class you're saving to isn't z-aware. If a feature class isn't z-aware then trying to save a geometry with z-values will throw an error.

feature.Value(0) = IDvalue Where are you declaring "feature"? As far as I can see it's not being declared (which would explain the object not set error) and you don't need it. If it is supposed to be an IFeature object then that would be incorrect as you can't add a feature to a table. Also, the field at index 0 is usually the ObjectId field. Is this true for your table? If so, the ObjectId field is readonly. If you try to set its value then you will get an error. ObjectIds are managed by the database.
