Can you try a tool like HttpFox to see what requests are being sent from Firefox and if any of them are failing? It may be that you don't have a crossdomain.xml file on your ArcGIS server. ... View more

The Map can only be in one spatial reference. In the code you'd set the IdentifyParameters.spatialReference property.
