The basemap services are all cached, and not downloadable. However, Esri does offer a product called Data Appliance for ArcGIS, which contains the same basemap services. You can read more about the Data Appliance here -http://www.esri.com/data/basemaps.html.
No, it is not possible to download buildings from the World Topo Map as they are just cached jpeg images. If you wish, you may contact the contributor and see if the buildings are available.
I have a similar question about downloading the World Topographic Map. I really like the world topographic map, however, we have a very poor internet connection speed. Is there a way to over come this problem? Can portions of the map be downloaded locally? Is there a way to cashe the map that might speed up things? Maybe storing images during off peak hours.

I love that we solved the problem of providing a great base map (that was our goal), and you�??re right on, now the next challenge is to make it more configurable.
