If you have the .dwg file, you can load it into ArcMap.
Set your page layout size and orientation as per the dwg.
Then convert the relevant titleblock pieces (i.e. the polylines) to graphics by right clicking on the feature in the Table of Contents.
In Data View, turn off the original dwg layer, select all the graphics and click copy.
Go into Layout View and paste outside of the data frame. The features should appear close to the page extents.
Some minor tweaking may be required to fit the features to the page.
