Error message when trying to add a layer from file.

The error message states "An error occurred while creating features. (Duplicate field names are not allowed. ())"  The file is a list of names, addresses, cities, zip codes and telephone numbers and saved as a .cvs file.  I am new at this but don't find any duplicates filed names, although some last names are the same.  Any thoughts?

Maybe you have a reserved field name that is used by arcmap... you might want to provide a list.  Perhaps fields like ID come to mind

Adding to Dan, also ensure the intended FieldNames shouldn't have space in them. 

e.g.

Tel Nos (Incorrect)

Tel_Nos (Correct)
