Something interesting has been happening with my editor toolbar and any custom tools that I add. Every time I turn off my machine, upon rebooting, my editor toolbar has been turned off and reset. All of the custom tools that I have added in have been removed as well.

The interesting thing is, I am using 10.1 Home Use as well as 10.1 Advanced on a separate machine in my office. This occurs on both machines. I can restart the program and all is well, but if the computer is turned off, that is when the reset occurs. Any ideas?

I am having a similar issue, I recently upgraded all of the PC's (6) in our office to 10.1, only one PC has an issue with the Editor Toolbar.  For some reason, the Editor Toolbar will not appear when activating it through the 'Toolbars list' or the 'Editor Toolbar Button'.
I am able to create a custom toolbar with most of the editor tools, but i cannot replicate the entire editor menu, and it will not auto-prompt the 'create feature templates' window to open.
I know it is a minor inconvenience, but is there a fix?? or possibly a file that was excluded during installation that i can drop into the <bin> folder?

I too recently upgraded to 10.0 and spent a couple hours trying to figure out why my toolbar customizations wouldn't stay put.  I can't remember exactly what I did, but I know it had to do with unchecking a certain option in the Customize window.  Check out Customize -> Customize Mode... -> Options tab.  There you will see an option 'Create new toolbars and menus in the document' and 'Save all customizations to the document'.

I had to play around with this and the toolbar positioning before I finally got it looking right.  Not sure if this answers your question, but I hope it steers you in the right direction.
