Because I have both Microstation and ArcGIS, I was able to find the coordinate location (start x,y and end x,y) of a 2 point line that crosses my general spatial work area.
I found the x,y coords in ArcGIS and I found the same x,y coords in Microstation and then came up with a "world" file.
"esri_cad.wld"
-1853483.648,-1789483.648 290000.000,5654000.000
-1447483.648,-1456483.648 696000.000,5987000.000
This world file works for me (UTM Zone 10, NAD83) but I doubt it would work for most other folks.
This issue has been identified as a "bug" (by ESRI) in ArcGIS 10.2

I exported to a DXF and it showed perfect when I brought it into Arc. Remember to set your projection.
