Survey123 creating Workforce duplicate Assignments after recent update

Hello!  I recently installed the latest Enterprise patches for our 10.9.1 Enterprise.  Since then if an assignment is created from Survey123 and assigned to a workforce user, if the user has an Android device and completes the assignment it creates a duplicate assignment to the second the original assignment is completed. The duplicate has the created username of the user that marked the assignment as complete. This just started happening last week. Has anyone come across this?  It is only on Android devices and only when the assignment is generated using Survey123.  Very strange.  This workflow had no issues before.  I do see a lot of replica sync failures in the server but even if I turn on verbose logging it gives no info other then failure in the log.  Thanks!
