Public Notification Widget - Labels Not Multi-Line

I am having an issue with my resulting PDF label sheets. I have formatted my popup in the addressee layer like this:

However, the resulting PDF Label sheets always have a single line of text in each label, instead of separated out into 3 lines like the popup. What am I doing wrong? I would appreciate any help!

Got it to go multi-line by viewing and adding <br> into the HTML via the Custom Attribute Display dialog.
