Edits to point features not saving.

I created a web app that allows users to edit inspection data (eg: changing the status, adding dates, etc) but when the users attempt to change those statuses the edits do not stick. I have tested in the web map that the web app looks at and get the same behavior. The point feature is filtered and the symbology looks at the status to change colors. This web app has worked for a couple years, but now this behavior is happening.

Are you getting an error with this? Or is it saying it 'saved' and when you check later it hasn't?

There is no error message, once I zoom out the edits revert back to what they were when the inspection was submitted.
