Create a new record icon is not visible in SMART EDITOR in Web AppBuilder

I have 2 feature classes and 2 related tables
- line A (feature) related to table A (table) with one-to-many relationship.
- Point B (feature) related to table B (table) with one-to-many relationship.
I am able to add a related record in table B as the icon for adding a related record appears in the SMART EDITOR. I am unable to see the icon when I want to add a record in Table A. The SMART EDITOR window does not show any related records for line A. I have checked the settings of my SMART EDITOR widget and the option to view all related tables and records is checked.
The popup for line A sometimes shows 'no related record' in different browsers. However, there is definitely a record in Table A for each feature in Line A. This issue is not consistent, so I am unable to find the cause of this issue. Has anyone faced something similar?
