Sub Layer Arrows do not show in Layer list

I have a flex viewer version 2 website that's been running for a while.
All of a sudden, the Arrows to display the Sub-Layers are not showing.
This is happening on multiple computers, so I don't think it's a Browser issue.
Does anyone know of anything that could be causing this?

   Have you checked the actual REST Service directory for this MapService and made sure the all the child layers are listed there?
