When publishing form on Survey123 Connect to our Portal 10.4 - I keep getting Error 500 Service Hosted/service.... Feature Server not found

I would appreciate if someone could help me with this problem.

When I try to publish my form on Survey123 Connect to our Portal 10.4 I get this Error 500 Service Hosted/service.... Feature Server not found .

I have already created and published many forms successfully but now I can´t anymore. I deleted all my forms but keep getting this message.

I'm getting error codes 502, 503, and 504 at the same step when trying to publish to Portal.

Please provide error logs and XLSForms in this thread or send to barmstrong at esri.com

Brandon, thanks for answering me.

I attached the XLSForm (Declaracao) for you to have a look.

About the error log, I don´t know how to get to it. I am trying to find how and where I can get it. 

Is it on ArcGis Server ??
