Uncaught (in promise) ChunkLoadError: Loading chunk 3 failed.

I'm not sure how this happened or how to  fix it. I'm working in Survey123 Connect. I've created this survey a while ago. But suddenly the web form is not loading. The loader never goes away. I don't see any errors in in Connect and it also publishes without errors.

Not sure if this helps but here's what I'm seeing in FF dev tools:

I had to give up and use the same XLSForm to create a new survey. Too many moving parts...
