The recent update to Survey123's web viewer styling is causing some confusion amongst users and myself as well.

The formatting of choice questions in particular is causing issues:

From the above example it seems as if both options are already selected and disabled causing confusion when filling in a survey.

Your assistance is greatly appreciated. Keep up the good work. @JamesTedrick

@DeonLengton ,

Can you please confirm that this is a survey published from a previous version of Survey123 Connect (<=3.11) after the recent February 2021 release ? We have a known issue for this case and planned a fix in a coming hotfix. Turning off the version lock setting for the web app or republishing the survey by using 3.12 Survey123 Connect should solve this issue.

  • Please be advised that after the fix, you still need to republish the survey by using your original version of Survey123 Connect to fix this.

If you encountered other cases, please let us know.

Hi @DeonLengton

Just FYI, the issue has been fixed now. New surveys published by any version of Survey123 Connect should work as expected now.

Thank you again for the feedback!

For anyone who concerns already published surveys that have this issue, republishing the survey by using the original version or the latest 3.12 version of Survey123 Connect can fix the issue.

Thanks,

Zhifang
