Did you resolve this issue? The sharing permissions and way surveys are displayed in mobile devices and desktop platforms is exactly the same. Is the user signed into the same account between the mobile device and desktop? Are they using the same version?

Phil, yes it's fine now. I noticed that day ArcGIs Online server was slow. I guess it took some time to update my new user groups. After 1 hour, without doing anything it, he was able to see and use the form. Thanks
