Yes, this is expected behaviour. Survey123 Connect will generate a warning if you enable both the Inbox and Sent folder at the same time that expands on this and recommends against it.
The survey instance can only exist in one of the two folders. If you refresh the Inbox and the response is in the sent folder (i.e. was submitted from that device), it does not get downloaded.
The solution in this case is to disable the sent folder. Users can update the 'sent' survey via the inbox and collaborate with other users.

Is there a need for the sent folder in your app? I turn mine off because of the confusion. But I agree that the Inbox should be the priority.
