This appears to be an ArcGIS Server error.  Can I ask that you open a support ticket to address this?

I am seeing the same error in a form I am designing. I, as the designer, am not seeing the error, but other users are seeing it. Maybe it happened after I changed the name of the form, but I am not sure.
