Survey 123 Issue - mobile device camera will not open

Wondering if anybody has come across this issue:

I created a simple form in Survey123 for citizens to report hazardous ice conditions. I added a camera capture question, made it required, and selected camera only, and set the min uploads to 1 and max to 4 images. 

I attempted to complete the survey, and when I would get to the camera question the form would open my image gallery, but not engage my camera. The phone is a Samsung S21 Ultra. 

 

I'm not sure if anyone else has experienced similar issues, or has any workarounds. I attached a screenshot showing all the settings used in the form design. 

 

It would be really nice to have this setting working properly, as it would be essential in my deployment to ensure users are submitting an actual camera photo from the time of their report, and not attaching a saved image from their gallery.
