Survey 123 | Error Code 0 | Unable to send completed survey

When trying to send a survey after completion, I get an error message saying "This survey was not able to be sent due to the following error: Code 0

We are running on ArcGIS Portal here at Arkansas Electric Cooperatives. We have this going through a hosting server. Any ideas on what could be the problem?

While the issue the original user was facing seemed to be related to settings with the Enterprise system etc., we are using AGOL and Survey 123 and a user in the field is getting a generic "Send Error" when connected to cellular and not wifi. I attempted to replicate the issue (field user and I are both using Apple devices) and when I was connected to wifi, the form sent fine, but over cellular I got the same generic Send Error. The field user is continuing work and storing the completed surveys in his outbox for now and will try to send when connected to wifi.

My question is what could be causing this? Shouldn't the completed forms be able to be sent over cellular?
