Send Error: Layer not found in feature service

I've got a brand new survey created in connect v 3.0.127 that just doesn't seem to be working no matter what I do.

Any time I attempt to send, it errors with the following dialog. The layer exists on our Portal just fine. Has anyone else experienced this issue, or can point me in the right direction?

Can you provide a screenshot of your Feature Service endpoint (/FeatureServer) ?
