Publishing Error:400 Code

Hey,

I am getting an error when trying to publish my survey. I am getting a 400 code that says unable to add feature service definition. I don't see any repetition in my titles but I could be looking over something. I'll attach the error image and my excel sheet. Anything helps, thank you!

Hi Zach,

I had a quick look at your xlsx file. The reason you are getting the error on publish is you have duplicate field names in the name column, so the add to service definition fails. Field names must be unique.

Please check and remove all duplicates and republish again. Note that a folder and feature service may have now been created in your Content folder in AGO or Portal. So you need to delete this folder and FS first, before trying to republish again.
