Does the issue happen every time they use the app?

Yes – the App crashes every time the user selects the WPU

please find the attached images of the error and excel document. the survey was working properly but now it has a bug.

Hi @DumisaniNkosi,

It appears you are using the web app, not field app, based on screenshot, but you are creating the survey in Connect. Can you send the XLSForm (xlsx file and any files in media folder) so we can see if we can reproduce it?

What version did you last publish the survey from Connect in, and do you use version locking for web form?
