Getting Operation Rolled back error when trying to send survey

I keep getting the message Operation rolled back when trying to submit my survey. It was working fine until I added jr:choice -name calculation fields. I added these to generate a column with the labels rather than names for the species select_one questions.  I am using version 3.24.30 of the Survey123 Connect and version 3.24.21 of the Survey123 mobile app. There are no updates available. 

How to fix this issue? Or is there a better way to get output to display labels instead of names? Excel sheet is attached.

I'm not sure what the issue is, but I usually fall back on S123 diagnostics when these types of things happen.  Check out this help article, under the section "Enable diagnostic logging".

The logs can be pretty verbose and it might not all make sense, but I've often discovered additional error details in there that help track down the issue.

You several fields using the same field name Species.  It should be flagging this with a green mark in the cell.  Cannot dup field names.

I have removed all the calculation fields and am still having the same problem. Attached is the new form I'm using and the error log. Can any Esri staff help out with this? I really need to get it fixed. TIA!

You have several name issues.

First Date is a reserved word.  See the green mark here telling you.

Here you have a space after the repeat name.  Again cannot ignore the green boxes on the name column.

For your species list just an idea but what we do is have species code scientific name  common name with spaces in the label section so crews can look up any of the names they know.

Start with these fixes and see.  Hope that works.

Thank for your suggestion @DougBrowning, I will work on those. A lot of them are for select_one questions and I'm struggling to figure out my these are flagged given the list names are correct. Do you have any ideas why this is? Thanks

select one and multiple are a special case as there is no way to program that in excel so they are fine.  I know that is a bit weird but they did the best they could with excel.  So yes you just have to know which are ok to be green.
