Error: Initializing form

Hello, 

I am running into an issue where my csv data is failing to load in the web form and when editing the data.

The csv is within the survey's folder. 

In Survey123 Connect the link appears like: 

Example of use in survey: 

pulldata('Valves','Note1','ValveID',${ValveID})

Web Form Error:
Error: Initializing form...

• Failed to load data from jr://file-csv/ .csv

Data Edit Error: 

Error: Initializing form...

• Failed to load data from https://www.arcgis.com/sharing/rest/content/items/79c2b1b49dc146808944f3c909172c5e/info/media/%20.cs....

Version (From Survey123 Connect Excel): 

This works in Survey123 Connect and in the Survey123 App. 

I am not aware of what is causing this issue. Any help would be apricated.
