Creating a report error 101

I have a survey built with survey123 connects; I am currently trying to generate a report template. When I click "create sample template" I get the error 101: Item specified by surveyItemId does not exist or is not accessible.null. There have been no changes to the survey, I tried republishing it and that didn't work. When I made a test survey in 123 online I was able to use the report feature and generate a sample report. Can someone please help? To me it seems like it won't work because it was built out in connects instead of online.

  • Is this survey hosted in ArcGIS Online or ArcGIS Enterprise?
  • Are you the owner of both the survey form item and its underlying feature layer?
  • If you open the survey in survey123.arcgis.com, does it load normally under the Collaborate and Analyze tabs?
  • Do any other users in your organisation get the same error when attempting to create a sample template?
